Mathematical Modeling on Alveolar Flooding and Its Fluid Clearance

Abstract

Pulmonary edema would cause ARDS (adult respiratory distress syndrome), and the reason can be cardiogenic or noncardiogenic. Cardiogenic pulmonary edema is because of the increase of pulmonary venous pressure. Noncardiogenic pulmonary edema is mainly because of attack of virus. Pulmonary edema is frequently accompanied by alveolar flooding due to malfunction of alveolar fluid clearance (AFC). Here a mathematical model consisting of 10 ODE's is set up to study the physiological and pathological mechanism of alveolar flooding and AFC. The ODE model consider three compartments: alveolar fluid domain, type I alveolar epithelial cell domain (ATI), and interstitium domain governs the dynamics of membrane potentials, ion concentrations and water transport in/between all compartments. ENAC and CFTR ion channels are considered on the apical membrane of ATI, and CaKC ion channel, NKCC1 cotransporter and Na-K-ATPase pump are considered onadult respiratory distress syndrome the basolateral membrane of ATI. Bifurcation analysis of conductance of ion channels, transporter and pump is studied. The results suggests blocking ENAC would cause alveolar edema and even alveolar flooding, which agrees well with experimental observations. Also, the results suggest that by blocking CFTR, CaKC and NKCC1, alveolar edema and flooding can be cured when they happen.
